About the position

The AOD Case Manager at Community Support Services Inc. is responsible for providing recovery-focused community rehabilitation services to individuals dealing with alcohol and other drug (AOD) issues and severe mental illnesses. This role involves coordinating care, monitoring service delivery, and assisting clients in developing coping skills and social support networks. The position requires collaboration with a multidisciplinary team to create and implement individualized service plans, ensuring clients receive the necessary support to manage their conditions effectively.

Responsibilities

  • Serve on a team of Case Managers, Recovery Specialists, and medical staff to address client needs and care.
  • Monitor and coordinate necessary evaluations and assessments to identify client needs.
  • Assist individuals in minimizing the effects of AOD and psychiatric symptoms to enable self-management.
  • Coordinate the development of an Interdisciplinary Service Plan (ISP) that includes necessary services and assistance.
  • Re-assess client strengths, needs, abilities, and preferences as required by ISP standards.
  • Connect clients with community resources to develop interpersonal and community coping skills.
  • Assist individuals in fostering and increasing their social support networks.
  • Accompany individuals to appointments and activities, providing daily living skills training as needed.
  • Ensure residents comply with occupancy rules established by relevant authorities.
  • Maintain relevant documentation.

Requirements

  • Minimum of a Bachelor's Degree in a human service field required.
  • Chemical Dependency license preferred.
  • Prior experience in case management with individuals with substance abuse required; experience with severe mental illness preferred.
  • A car and a valid Ohio driver's license required, with insurability through the Agency's insurance carrier.
  • A belief in CSS's recovery-centered mission.
  • Knowledge of the Housing First philosophy.
  • Ability to develop positive relationships with individuals facing mental illness and substance abuse challenges.
  • Skill in assessing individuals for strengths, needs, abilities, and preferences.
  • Excellent organizational, attention to detail, and communication skills.
  • Ability to work in a team environment.
  • Proficiency in Microsoft Outlook.
  • Skill in managing time and meeting deadlines.
  • Ability to negotiate assertively from an advocacy perspective.
  • Knowledge of community resources.
