About the position

ProHealth Physicians is seeking a Primary Care Advanced Practice Clinician (PA or NP) to join their team in Essex, Connecticut. This role is integral to the Optum Care Delivery team, focusing on providing patient-centered, team-based primary care in a supportive environment. The position emphasizes autonomy in practice while collaborating with other clinicians, and it offers opportunities for growth and leadership within a Fortune 5 organization.

Responsibilities

• Provide direct patient care in a primary care setting.
• Educate and empower patients to take ownership of their health.
• Collaborate with other clinicians in a team-based practice.
• Participate in an onboarding and mentorship program.
• Manage a dedicated patient panel with moderate scheduling templates.

Requirements

• Master's degree in Nursing (APRN or FNP) or Physician Assistant program.
• Unrestricted Nurse Practitioner / Physician Assistant license in CT (or ability to obtain).
• Certified as a Family Nurse Practitioner through the ANCC/AANP or certified as a Physician Assistant through the NCCPA.
• Unrestricted CSR and DEA license (or willing to obtain prior to start).
