About the position

The position involves the debridement of wounds using various methods such as sharp, enzymatic, autolytic, or surgical techniques that can be safely performed during patient visits. The role includes the application and management of topical wound care and associated products, as well as directing and recommending services to enhance wound healing outcomes. This encompasses a range of services including labs, radiologic tests, vascular tests, referrals, home health care, and the use of devices like Vacuum Assisted Closure and orthotics. The position also requires the interpretation of diagnostic tests and making appropriate follow-up recommendations. Additionally, the role involves assessing diabetic foot ulcer risks for diabetic patients according to ADA guidelines, educating patients on wound management, and providing updates to primary care physicians regarding their patients' conditions. The individual will act as a resource for health-related decisions and collaborate with staff to identify and meet customer needs.

Responsibilities

  • Debridement of wounds by sharp, enzymatic, autolytic or surgical methods.
  • Application and management of topical wound care and associated products.
  • Direction and recommendation of associated services to improve wound healing outcomes.
  • Interpretation of diagnostic tests and recommendation of appropriate follow up.
  • Recommendations for prescription of required medications and/or Durable Medical Equipment (DME).
  • Diabetic Foot Ulcer risk assessment for Members with Diabetes.
  • Education of patients on topical and systemic management of their wounds.
  • Education of the Primary Care Physician (PCP) on current wound management practices.
  • Provision of regular updates to the PCP on the status of patients' systemic conditions.
  • Acts as a resource person for decisions, planning, and policies affecting health.
  • Collaborates with staff to identify customer needs and develop plans to meet these needs.
  • Other duties as assigned.

Requirements

  • Registered nurse (RN) preparation and Nurse Practitioner (NP) license required.
  • Minimum of 1-year experience as an NP.
  • Minimum of 5 years' experience as an RN.
  • Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
