About the position

The Program Director for the Intensive Community Wrap Around Supports program at Open Sky Community Services will play a pivotal role in overseeing a dedicated team that provides essential services to adults aged 18 to 30 with Autism Spectrum Disorder and Serious Mental Illness. This position is designed to ensure that individuals receive the necessary support to maintain stability and functionality in their homes and communities. The Program Director will be responsible for supervising a multidisciplinary team that includes clinicians, nurses, team coaches, consultants, and peer mentors. This role requires a commitment to trauma-informed care and the application of evidence-based practices to foster individual growth and community integration. The Program Director will facilitate daily communication among team members to enhance collaboration and planning. They will manage all referrals for the Community Treatment Team, ensuring that tracking, responses to funders, and maintenance of active rosters and wait lists are handled efficiently. A critical aspect of this role is to ensure that all required clinical assessments are scheduled and completed within 30 days of admission, providing a blend of in-person and virtual support to meet the needs of individuals served. Additionally, the Program Director will develop and participate in an after-hours on-call rotation to promote community access and stabilization for individuals in need. This position also involves administrative responsibilities, including the accurate submission of billing and program expenditure documentation within required timelines. The Program Director will be expected to provide necessary training for the clinical treatment team on various topics, including Autism Spectrum Disorder, significant mental illness, and evidence-based practices such as PACT, DBT Informed Skills system, Motivational Interviewing, Harm Reduction, and PBS. This role is crucial in ensuring that the program meets its goals and effectively supports the individuals it serves.

Responsibilities

  • Supervise and provide oversight to the Intensive Community Wrap Around Supports team including clinicians, nurses, team coaches, consultants, and peer mentors.
  • Facilitate a daily communication forum for all team members to collaborate, plan, and take action.
  • Manage all referrals for the Community Treatment Team, including tracking, responding to the funder, and maintaining active rosters and wait lists.
  • Ensure all required clinical assessments are scheduled and completed within 30 days of admission.
  • Provide a combination of in-person and virtual support modalities to meet the comfort of all individuals served.
  • Develop and participate in the after-hours on-call rotation to promote community access and stabilization for individuals served.
  • Submit billing and program expenditure documentation accurately and within required timelines.
  • Provide necessary training for the clinical treatment team on Autism Spectrum Disorder, significant mental illness, and evidence-based practices.

Requirements

  • Master's Degree in Social Services field with current licensure (LMHC, LICSW, BCBA or higher) or license eligibility.
  • Minimum of five years direct experience with individuals with Autism Spectrum Disorder and Significant Mental Illness.
  • Direct experience implementing evidence-based practices.
  • Valid driver's license, acceptable driving record, and reliable vehicle.
