About the position

NorthKey Community Care is seeking a full-time Assertive Community Treatment (ACT) Case Manager to work with clients suffering from serious mental illness (SMI). This role focuses on providing case management and care coordination services, empowering clients to develop necessary skills for recovery and self-management. The position is based in Covington, Kentucky, with potential travel to other locations.

Responsibilities

  • Engage clients with serious mental illness (SMI) in a structured, collaborative relationship to empower them in recovery and self-management.
  • Promote environmental and psycho-social health to maximize independence and improve lifelong wellbeing.
  • Provide evidence-based care for complex cases of vulnerable individuals with SMI.
  • Address functional challenges faced by individuals served.
  • Identify and mitigate risks and gaps in care to prevent deterioration that could lead to hospitalizations or loss of independence.
  • Complete requests for prior authorizations for necessary services.
  • Participate in developing individualized interdisciplinary care plans for clients.
  • Educate clients to promote improved functioning and skills development.
  • Complete mandatory training related to care coordination and case management.
  • Collaborate with other care team members involved in the client's case.
  • Maintain HIPAA compliance and client confidentiality.
  • Educate clients and families on available resources to assist with identified needs.

Requirements

  • Bachelor's degree in a human services or behavioral health field from an accredited institution.
  • Experience working with individuals with serious mental illness (SMI) is preferred but not required.
  • Excellent written and verbal communication skills.
  • Valid driver's license and ability to operate a motor vehicle.

Nice-to-haves

  • Experience in case management (1 year preferred).
  • Graduates with a Bachelor's degree in May 2024 or December 2024 are encouraged to apply.
