About the position

The Assessment & Quality Coordinator at UCP Oregon plays a crucial role in supporting adults with disabilities by ensuring the quality of services they receive. This position involves conducting assessments, collaborating with personal agents, and implementing quality assurance measures to enhance service delivery. The coordinator will work closely with customers, families, and the brokerage team to foster a culture of transparency and accountability while promoting equity and inclusion in service provision.

Responsibilities

  • Evaluate and oversee the quality of services provided to customers by their designated Personal Agents.
  • Conduct the Oregon Needs Assessment to determine service hours and rates for customers.
  • Facilitate the assessment process from beginning to end, addressing discrepancies and resolving complaints.
  • Act as the primary liaison with customers, families, and other stakeholders regarding assessment purposes and results.
  • Implement quality assurance measures informed by customer feedback and advisory boards.
  • Foster a culture of transparency and accountability within the brokerage team.
  • Incorporate trauma-informed and equity practices into quality improvement measures.

Requirements

  • Bachelor's degree in social services or a closely related field with at least 2 years of relevant work experience; or an Associate's degree with at least 3 years of experience; or a high school diploma/GED with at least 5 years of experience.
  • Valid driver's license, reliable personal vehicle, and a clean driving record.
  • Ability to pass a reference check and criminal history background check.
  • Demonstrated commitment to customer self-determination and personalized support.
  • Strong prioritization and project management skills in a fast-paced environment.
  • Effective written and verbal communication skills with diverse constituencies.

Nice-to-haves

  • Fluency in a second language, including ASL.
