About the position

The Assessment and Service Coordinator at Peninsula Volunteers Inc. plays a crucial role in supporting the aging population by conducting assessments for the Meals on Wheels program. This position involves evaluating client eligibility, providing information and referral services, and maintaining accurate database records. The coordinator is expected to embody the organization's core values of respect, commitment, trust, dedication, and compassion while working collaboratively with clients and team members.

Responsibilities

  • Complete in-home and telephone reassessment of PVI MOW clients as required by San Mateo County Aging & Adult Services contract.
  • Provide information and referral services to clients and maintain updated database records.
  • Perform reassessments of client's eligibility for participation in Meals on Wheels by completing evaluations in clients' homes and by telephone at quarterly intervals.
  • Conduct initial intake of new clients.
  • Connect clients with appropriate resources based on individual client needs.
  • Communicate with all customers, including clients, family members, social workers, Aging and Adult Services staff, contractors, etc. as needed.
  • Document communication in writing in client file.
  • Update and support client database.
  • Answer phones in office, return calls, and other related administrative duties as assigned.
  • Assist in Meals on Wheels report preparation.
  • Work cooperatively with members of the team to serve program participants and volunteers in a comprehensive, effective, and caring manner.

Requirements

  • A.A. degree minimum, BA preferred (Nutrition, Health Science, Social Work, Gerontology).
  • Experience with senior population or social service background.
  • Computer/data entry knowledge.
  • Proficiency in MS Word, Excel, Outlook.
  • Strong oral and written English communication skills.
  • Spanish speaking helpful.
  • Valid California Driver's License; maintain a good safety record (verified by DMV at hire and periodically) and a reliable vehicle; proof of liability insurance with a minimum of 100,000/300,000 coverage.
  • Criminal background clearance is required.
