About the position

The Assessor Reviewer II is responsible for conducting face-to-face assessments of vulnerable populations in Washington, DC. This role involves evaluating individuals in their homes or community settings, documenting assessment results, negotiating service plans, and ensuring appropriate care through communication with healthcare providers. The position also includes mentoring new staff, conducting quality assurance competency reviews, and participating in process improvement initiatives.

Responsibilities

  • Perform face-to-face assessments of DC residents.
  • Document assessment results for individual member status.
  • Negotiate service plans as needed.
  • Complete quality assurance competency reviews and other assigned tasks/projects such as new assessor training.
  • Provide ongoing education to staff and stakeholders.
  • Communicate with all health care providers to ensure appropriateness of care.

Requirements

  • Four-year degree or master's degree in nursing and/or social work.
  • Minimum of three years of experience in a relevant field.
  • Registered Nurse (RN) or Licensed Independent Clinical Social Worker (LICSW) with a current and unrestricted District of Columbia license.
  • Case management and Medicaid experience preferred.
  • Experience completing level-of-need assessments preferred.
