About the position

The Arbors Assisted Living is looking for a compassionate and motivated Case Manager to coordinate the community's case management system. This role is essential in ensuring that each resident maintains their well-being and achieves the highest possible level of physical, mental, and social health. The position requires flexibility in working hours, including nights, weekends, and holidays, and offers significant potential for professional growth.

Responsibilities

• Coordinate the community's case management system.
• Monitor and maintain awareness of each resident's well-being.
• Ensure residents attain the highest practicable level of physical, mental, and social well-being.

Requirements

• Bachelor's or Master's degree in human resources or service delivery from an accredited college or university.
• One year of full-time experience in providing services to a dependent adult population, or an Associate's degree with three years of experience.
• Two years of experience in a related field.
