About the position

The Assisted Living Coordinator - LPN is responsible for overseeing the health and personal care needs of residents in an assisted living facility. This role involves direct supervision of nursing staff, ensuring compliance with health regulations, and managing resident assessments and care plans. The position requires a hands-on approach, as the coordinator will also work as a floor nurse, providing care and support to residents while ensuring their safety and satisfaction.

Responsibilities

  • Directs the health concerns of each Assisted Living Resident, including assessments and personal care needs.
  • Hires and supervises nursing staff for the assisted living unit.
  • Ensures services are performed with consideration for residents' health, welfare, and satisfaction.
  • Collaborates with the Director of Post-Acute Services regarding clinical operations.
  • Completes pre-admission screens for residents prior to move-in.
  • Monitors residents for changes in condition and addresses them promptly.
  • Directs Lifestyle and Care Needs Assessments for residents.
  • Assists residents with Activities of Daily Living (ADL) such as bathing and grooming.
  • Updates CareTracker information and ensures timely documentation.
  • Leads the development of service plans in conjunction with residents and families.
  • Facilitates Assisted Living Resident Meetings and addresses feedback.
  • Coordinates pharmacy and medication needs, ensuring compliance with regulations.
  • Acts as a liaison between residents, health campus, and external healthcare providers.
  • Supervises treatment regimens per physician orders.
  • Maintains resident medical records according to state guidelines.
  • Trains Nursing Assistants in ADL assistance and health-related training.
  • Supports the hiring and training of clinical staff in the Assisted Living unit.
  • Ensures CPR/First Aid training meets state regulations.
  • Serves as weekend management and provides on-call coverage as needed.
  • Assists in organizing marketing programs for the campus.
  • Participates in various committees of the health campus.

Requirements

  • LPN with a degree from an accredited school of nursing.
  • Current unencumbered LPN license in the state of practice.
  • Valid CPR certification.
  • Valid driver's license and ability to operate a large van/bus.
