The Associate Chief of Staff, Geriatrics, Palliative and Extended Care (ACOS, GPEC) at the San Francisco VA Health Care System (SFVAHCS) is a pivotal role that reports directly to the Chief of Staff (COS). This position serves as the system's executive expert in geriatrics, palliative, and extended care matters, with comprehensive programmatic responsibility for planning, developing, and directing the administrative and operational activities of the Geriatrics, Palliative and Extended Care Service. The ACOS, GPEC oversees a variety of clinical programs, including an inpatient Mobile ACE program, geriatrics consult service, and the medical and recreational therapy care provided in the Community Living Center (CLC), which has 120 beds dedicated to long-term and short-term nursing care. Additionally, the role encompasses management of a dedicated hospice unit within the CLC, a Home-Based Primary Care program, inpatient palliative care consult service, outpatient palliative care clinic, and outpatient primary and consultative geriatrics clinics. The ACOS, GPEC also manages an extensive non-institutional care component that includes Adult Day Healthcare (ADHC), Homemaker Health Aide services (HHA), In Home Respite (IHR), and Community Nursing Home (CNH) care, serving Veterans across multiple community-based outpatient clinics. The position is integral to the educational programs at SFVA Medical Center, including the UCSF/SFVAHCS Geriatric Medicine Fellowship Program and the Integrated Geriatric and Palliative Medicine Fellowship Program. In terms of administrative responsibilities, the ACOS, GPEC is tasked with providing oversight and leadership within the Geriatrics, Palliative and Extended Care Service, ensuring high standards of excellence across all areas. This includes championing performance measures related to wait times, missed opportunities, and outpatient satisfaction, as well as being directly responsible for the recruitment and retention of faculty and staff, and initiating programmatic development. The role also requires maintaining a clinical workload by providing direct patient care as needed in various settings, including inpatient, nursing home, or ambulatory care, utilizing both face-to-face visits and telemedicine. The ACOS, GPEC must meet all customer service, age, development, cultural needs of patients, and computer security requirements.
