The Associate Chief of Staff for Geriatrics, Palliative and Extended Care (ACOS, GPEC) at the San Francisco VA Health Care System is a pivotal leadership role that reports directly to the Chief of Staff (COS). This position serves as the system executive expert in geriatrics, palliative, and extended care matters, with comprehensive programmatic responsibility for planning, developing, and directing the administrative and operational activities of the Geriatrics, Palliative and Extended Care Service at the SFVAHCS. The ACOS, GPEC oversees a variety of clinical programs, including an inpatient Mobile ACE program, geriatrics consult service, medical and recreational therapy care in the Community Living Center (CLC), a dedicated hospice unit, Home-Based Primary Care program, inpatient palliative care consult service, outpatient palliative care clinic, and outpatient primary and consultative geriatrics clinics. In addition to these clinical responsibilities, the ACOS, GPEC manages an extensive non-institutional care component that provides Adult Day Healthcare (ADHC), Homemaker Health Aide services (HHA), In Home Respite (IHR), and Community Nursing Home (CNH) care to Veterans across multiple community-based outpatient clinics. The role also encompasses oversight of primary GPEC educational programs, including the UCSF/SFVAHCS Geriatric Medicine Fellowship Program and the Integrated Geriatric and Palliative Medicine Fellowship Program. The ACOS, GPEC is tasked with establishing and maintaining high standards of excellence across all areas of the Geriatrics, Palliative and Extended Care Service. This includes oversight of performance measures such as wait times, missed opportunities, and outpatient satisfaction. The incumbent is also responsible for the recruitment and retention of faculty and staff, as well as initiating programmatic development. In addition to administrative duties, the ACOS, GPEC maintains a clinical workload by providing direct patient care as needed in various settings, including inpatient, nursing home, or ambulatory care, utilizing both face-to-face visits and telemedicine. The position requires adherence to customer service standards, cultural needs of patients, and computer security requirements.
