Associate Social Worker

About the position

The Associate Social Worker at Journey Health is responsible for providing high-quality clinical care to patients with diverse medical and social needs. This role focuses on the behavioral and psychosocial aspects of patient care, ensuring comprehensive plans that involve the patient's family or caregiver. The position requires maintaining a caseload of MediCal patients, facilitating access to social resources, and addressing mental health issues through evidence-based practices.

Responsibilities

• Maintain a caseload of MediCal patients for ongoing therapeutic and social support
• Facilitate patient access to social resources, including applying for Medicaid benefits and connecting with local community services
• Participate in case management reviews with the clinical team and advocate for patients' needs
• Address patients' mental health issues using evidence-based practices
• Utilize therapeutic techniques to manage mental health crises such as domestic violence and suicide ideation
• Complete assessments and plans of care related to patients' mental, behavioral, and social needs
• Collaborate with providers to support patients with treatment plans and medication adherence
• Demonstrate cultural sensitivity and competence when working with clients from various backgrounds
• Ensure compliance with federal, state, and local regulatory and reporting standards
• Perform other related activities as assigned.

Requirements

• Master's degree in social work from an accredited institution
• At least 1 year of experience in social services settings
• Ability to manage complex patients and navigate multiple projects
• Skilled in delivering a holistic view of health considering physical, behavioral, and social drivers
• Knowledge and skills to perform comprehensive social work assessments and develop actionable health plans
• Adaptability and proactivity in a dynamic environment
• Effective communication and problem-solving skills.
