About the position

The At-Home Physical Therapist at JAG Physical Therapy is responsible for providing high-quality rehabilitation services to patients in their home environment. This role emphasizes building strong relationships with patients to understand their unique medical, social, and functional histories, allowing for personalized and effective treatment plans. The therapist will work collaboratively with an interdisciplinary team to ensure optimal patient outcomes and will be supported by a company that values professional development and employee well-being.

Responsibilities

  • Collect and record patient care data following guidelines and regulations.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to formulate diagnosis and treatment plans.
  • Establish treatment plans based on evaluations and best practices.
  • Collaborate with interdisciplinary teams and patient support systems.
  • Assess and modify treatment plans as necessary.
  • Provide education and training to patients and their support systems.
  • Deliver safe and effective interventions in compliance with company standards.
  • Document all treatments and data using WEB PT EMR.
  • Present facility-level in-service annually.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements.
  • Valid CPR License.
  • Experience with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
