About the position

The At-Home Physical Therapist at JAG Physical Therapy is responsible for providing high-quality rehabilitation services to patients in their home environment. This role emphasizes building strong relationships with patients to understand their unique medical, social, and functional histories, allowing for personalized and effective treatment plans. The position requires a commitment to patient success and collaboration with an interdisciplinary team to ensure optimal care outcomes.

Responsibilities

  • Collect and record patient care data following regulatory guidelines and reimbursement systems.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer appropriate tests and measures for evaluations.
  • Synthesize evaluation results to formulate diagnoses, goals, prognosis, and treatment plans.
  • Establish treatment plans based on evaluations and best practices.
  • Collaborate with interdisciplinary teams and patient support systems.
  • Assess and modify treatment plans and goals as needed.
  • Initiate and complete discharge plans for patients.
  • Provide education and training to patients and their support systems.
  • Deliver safe and effective interventions in compliance with JAG Physical Therapy standards.
  • Document treatment data using WEB PT EMR for all services provided.
  • Present facility-level in-service annually.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.
  • Maintain a positive, professional demeanor at all times.

Nice-to-haves

  • Experience with EMR software.
  • Knowledge of various patient diagnoses and treatment protocols.
