About the position
The ATS Case Manager position at Mayflower Recovery involves providing confidential addiction counseling and case management services to individuals dealing with substance use disorders. The role requires motivating clients to stay in treatment and embrace aftercare recommendations while maintaining regular communication with their families. The Case Manager will work within a multidisciplinary team to ensure clients receive appropriate levels of care and support throughout their treatment journey.
Responsibilities
- Complete all clinical documentation for caseload including Biopsychosocial assessments, treatment plans, and individual progress notes.
- Develop discharge/transfer plans for patients to ensure appropriate levels of care.
- Contact referral sources and provide case consultation and coordination.
- Coordinate ongoing care for housing and therapeutic needs with the treatment team and community resources.
- Document all counseling and education services, assessments, and follow-ups in the patient's clinical record.
- Facilitate groups and provide substance abuse counseling services as specified in treatment plans.
- Review and revise treatment plans as necessary throughout the treatment episode.
- Participate in continuing professional development and maintain knowledge of medication-assisted therapy.
Requirements
- Previous experience with substance use disorder treatment and case management is required.
- Ability to assess, plan services, and deliver services to individuals in need of substance abuse treatment.
- Skilled in meeting patient needs in accordance with state required staff-to-patient ratios.
- Demonstrated knowledge of accepted principles of patient record management.
Nice-to-haves
- Knowledge of medication-assisted therapy.
- Experience in facilitating group counseling sessions.
