About the position

The Attending Physician role at Theoria Medical involves providing high-quality, patient-centered primary care services primarily in skilled nursing facilities. This position emphasizes flexibility in scheduling, allowing physicians to tailor their work hours to fit their personal lives while ensuring comprehensive care for residents. The role includes collaboration with Nurse Practitioners and Physician Assistants, with a focus on managing both acute and chronic illnesses, and utilizing advanced electronic health record systems for documentation.

Responsibilities

• Serve as the attending physician for residents at each facility.
• Collaborate with Nurse Practitioners and Physician Assistants for day-to-day resident care.
• Provide comprehensive primary care services by rounding on patients in skilled nursing facilities.
• Diagnose and treat patients with acute illnesses and chronic diseases.
• Complete provider documentation in Theoria's proprietary electronic health record system, ChartEasy™.
• Participate in Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives.
• Supervise collaborating nurse practitioners and physician assistants.
• Utilize electronic/online medical tools for administrative functions.

Requirements

• Medical degree (MD or DO) with active state licensure.
• Experience in primary care or related field.
• Ability to work collaboratively with a healthcare team.
• Proficiency in using electronic health record systems.

Nice-to-haves

• Experience in skilled nursing facilities or post-acute care settings.
