About the position

New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency located in Midtown Manhattan, with a satellite office in the Bronx. The agency is dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs of New York City. NAC is currently seeking a Bachelor's Level Case Planner to join its Special Medical Prevention Program. This program delivers a range of clinical, case management, and supportive services to families who are involved with child welfare and are caring for children diagnosed with chronic or complex medical conditions. The primary goal of the program is to ensure that families remain intact while providing caregivers with the necessary support to meet their children's physical, developmental, and mental health needs. The ideal candidate for this position will be strength-based, trauma-informed, culturally responsive, and committed to family preservation and resilience. This role requires the ability to work independently and flexibly on weekdays to meet the needs of youth and caregivers. The position operates within a hybrid model, allowing for work-from-home arrangements when staff are not in the field. This role offers a highly rewarding opportunity to build relationships with underserved families and to be part of an innovative multidisciplinary team that includes pediatricians, nurses, psychiatrists, psychologists, clinicians, case associates, and education and resource specialists. As a Bachelor's Level Case Planner, the individual will provide casework and clinical services to youth and caregivers, coordinating services for a caseload of 8-9 families as recommended by NAC's multidisciplinary treatment team. The Case Planner will conduct home and community-based visits, adhere to agency and ACS regulatory requirements, and collaborate with various professionals in a dynamic, strength-based interdisciplinary team model.
