About the position

The Behavioral Health Case Manager I is responsible for providing case management services to members with behavioral health and substance abuse needs. This role involves conducting assessments, developing care plans, monitoring their effectiveness, and coordinating with various resources to ensure members receive appropriate care. The position requires telephonic and/or home visit interactions with members, emphasizing the importance of clinical judgment and knowledge of community resources.

Responsibilities

  • Perform case management telephonically and/or by home visits for members with behavioral health and substance abuse needs.
  • Use appropriate screening criteria and clinical judgment to assess member needs.
  • Conduct assessments to identify individual needs and develop care plans.
  • Monitor and evaluate the effectiveness of care plans and modify them as needed.
  • Support member access to quality and cost-effective care.
  • Coordinate with internal and external resources to meet identified member needs and collaborate with providers.

Requirements

  • MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing with a minimum of 3 years clinical experience in social work counseling.
  • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist as applicable by state law.
  • Experience with complex psychiatric and substance abuse treatment.

Nice-to-haves

  • Experience in case management and telephonic/in-person coaching with members with complex psychiatric/substance abuse disorders.
  • 2 years post clinical licensure experience preferred.
  • Clinical telephonic experience.
  • Proficient use of Microsoft programs and ability to work with multiple computer programs simultaneously.
  • Knowledge of community resources and coordination.
