About the position

The Clinical Case Manager at CVS Health is responsible for facilitating appropriate member physical health and behavioral healthcare through advanced clinical judgment. This role involves conducting assessments, planning care, collaborating with providers, and coordinating psychosocial services to ensure optimal outcomes for members. The position requires travel within New Jersey and is primarily telework-based, adhering to standard business hours.

Responsibilities

• Conduct face-to-face visits in acute mental health facilities, members' homes, and the community.
• Execute comprehensive assessments of member needs and eligibility by evaluating member benefit plans and available services.
• Develop strategies to reduce risk factors and address complex clinical indicators impacting care planning.
• Perform crisis intervention for members experiencing behavioral health or medical crises and ensure follow-up services are received.
• Adapt clinical guidelines and case management procedures to ensure appropriate administration of benefits.
• Counsel with medical and behavioral health professionals to influence member care.
• Coach members to make independent medical and healthy lifestyle choices and participate in healthcare decision-making.
• Train and coach junior colleagues in techniques and responsibilities.
• Coordinate care with members, authorized representatives, and care team participants.
• Document accurately and timely in the electronic health record.

Requirements

• Independent New Jersey License: LCSW, LMFT, LPCC
• 3+ years of experience in behavioral health or social services
• 2+ years of experience with personal computers and MS Office Suite applications
• Must reside in New Jersey and be willing to travel up to 25-50%.
