About the position

The Behavioral Health Case Manager at AccessHealth Community Health Center is responsible for providing integrated behavioral health services to patients with identified mental health needs. This role involves assessing patients, providing mental health coping skills training, and collaborating with primary care and behavioral health teams to ensure comprehensive care. The case manager will also educate the community about mental health issues and promote healthy coping strategies.

Responsibilities

  • Accept referrals from primary care providers for patients in need of behavioral health services.
  • Initiate contact with new patients within one week of referral and follow up as necessary.
  • Screen and complete assessments for referred patients.
  • Work with patients on a 4-week basis to track progress towards identified goals.
  • Provide education about diagnoses and treatment options to empower patients toward self-care.
  • Assist patients with comorbidities such as diabetes and hypertension in meeting treatment goals.
  • Assess treatment adherence and address barriers to care.
  • Collaborate with primary care and behavioral health teams to meet patients' mental health needs.
  • Monitor patients for changes in clinical symptoms or treatment side effects.
  • Support psychotropic medication management prescribed by PCPs.
  • Document all patient contacts and treatment changes in the electronic health record.
  • Conduct consultations with the behavioral health team or psychiatrist for patients not adhering to treatment.
  • Participate in regular case staffing meetings with the behavioral health care team.
  • Continue patient contact for required sessions until recovery is determined.
  • Facilitate referrals for additional services as needed.
  • Inform supervisors of program progress and patient issues.
  • Collaborate with other AccessHealth departments to provide cohesive care.
  • Maintain professional relationships with community providers to foster coordination of services.
  • Comply with all company policies and regulatory requirements.

Requirements

  • Bachelor's degree in psychology, social work, or a mental health-related field.
  • Experience in a behavioral health setting.
  • One year of work experience in a relevant field.
  • High ethical standards and ability to maintain patient confidentiality.
  • Excellent communication skills and comfort with public speaking.
  • Ability to work with diverse populations and socioeconomic levels.
  • Strong organizational skills and ability to handle multiple projects.
