About the position

The Behavioral Health Case Manager will support clients dealing with mental illness and substance abuse issues by providing comprehensive case management services. This role involves developing and implementing client service plans, teaching essential life skills, and ensuring adherence to safety protocols. The Case Manager will work collaboratively with a team to facilitate recovery and promote community integration for clients.

Responsibilities

  • Support clients in psychiatric rehabilitation and medication management skills.
  • Teach symptom management strategies and relapse prevention techniques.
  • Assist clients with independent living skills development and health education.
  • Locate housing and provide budgeting assistance for clients.
  • Link clients to behavioral health resources and opportunities for community integration.
  • Communicate promptly with team members and community providers via various channels.
  • Develop and implement client service plans consistent with program philosophy.
  • Teach and assist clients in daily living activities, including shopping and meal planning.
  • Monitor and report on clients' general tone and mood, ensuring cultural sensitivity.
  • Implement agency safety protocols and act as a positive role model for clients.
  • Provide individual and group counseling, consultation, and case management.

Requirements

  • Must meet the requirements of a Mental Health Rehabilitation Specialist as outlined by Medi-Cal.
  • Associate Arts degree with 6+ years of experience in a mental health setting, or Baccalaureate degree with 4 years of experience, or Master's degree with 2 years of experience.
  • Experience with mental illness and substance abuse issues is preferred.
  • Fluency in English; bilingual in English/Spanish preferred.
  • Ability to work independently and maintain good boundaries and judgment.
  • Interest in working with persons with a behavioral health disability.
  • Ability to comply with agency rules, regulations, and policies.
