About the position

As a Behavioral Health Clinician/Case Manager at UnitedHealth Group, you will play a crucial role in facilitating and providing linkage to behavioral health services for the justice-involved population. This position is primarily based on-site at a Community Transition Center in San Diego, with minimal travel required (no more than 25%) to various probation offices and jails within the San Diego area. Your work schedule will be from Tuesday to Saturday, 9:30 AM to 6:00 PM. In this role, you will conduct comprehensive behavioral health screenings in dynamic environments, including jails and other settings. You will review available behavioral health histories to coordinate services and ensure that clients receive the appropriate level of care. Your responsibilities will include identifying and establishing contacts with community resources for the justice-involved population and guiding the coordination of care for post-release offenders to community behavioral health and ancillary services. You will also provide short-term clinical case management to ensure continuity of behavioral health and medical services. You will be expected to navigate and advocate within the behavioral health system on behalf of clients, engaging them in the screening process and collaborating with a multi-disciplinary team to develop case plans tailored to their individual needs and objectives. Utilizing motivational interviewing techniques, you will help clients identify their intrinsic goals and engage in behavior change. Additionally, you will participate in ongoing multi-disciplinary team meetings, providing education and consultation regarding linkage to behavioral health and ancillary services within the system of care. Your role will also involve guiding clinical care consultations with personnel from the California Department of Corrections and Rehabilitation, the San Diego Sheriff's Department, probation officers, and community treatment providers. You will assess safety risks, provide crisis intervention, and conduct behavioral health screenings for the population. Maintaining timely and appropriate clinical documentation records in the San Diego County Probation and Behavioral Health Electronic Health records is essential, as is adhering to HIPAA rules and regulations. Responsibilities

  • Conduct comprehensive behavioral health screenings in dynamic environments, including jails and other settings.
  • Review available behavioral health history to coordinate services and assure appropriate level of care for clients.
  • Identify, establish contacts with, and navigate community resources for the justice-involved population.
  • Lead coordination of appropriate level of care linkage for post-release offenders to community behavioral health and ancillary services.
  • Provide short-term clinical case management to ensure continuity of behavioral health and medical services.
  • Assist Behavioral Health System of Care (BHST) Care Advocates in daily operations and duties.
  • Advocate within the behavioral health system on behalf of clients as needed.
  • Engage clients in the screening process and collaborate with a multi-disciplinary team to develop case plans.
  • Utilize motivational interviewing techniques to help clients identify intrinsic goals and engage in behavior change.
  • Participate in ongoing multi-disciplinary team meetings providing education and consultation regarding linkage to behavioral health and ancillary services.
  • Guide clinical care consultations with California Department of Corrections and Rehabilitation personnel, San Diego Sheriff's Department, probation officers, and community treatment providers.
  • Troubleshoot internal and external problems as they arise.
  • Assess safety risks and provide crisis intervention and behavioral health screenings for the population.
  • Review clinical screenings, linkage, and levels of care requests.
  • Assist in orientation of Behavioral Health System of Care to probation partners.
  • Provide ongoing education of the behavioral health system of care to providers, consumers, and other stakeholders.
  • Suggest changes and improvements in the functioning of the department and coordination of care within the system of care.
  • Maintain timely and appropriate clinical documentation records in San Diego County Probation and Behavioral Health Electronic Health records.
  • Utilize technological systems to support clinical delivery of services, including access to Cerner Electronic Health Record and Probation Case Management System.
  • Adhere to HIPAA rules and regulations.

Requirements

  • Independently licensed, Master's level mental health professional (LMFT, LCSW, LPCC, etc.) or Psy.D in California, or an RN with psychiatric nursing experience.
  • 2+ years of experience providing direct clinical care in a behavioral health treatment setting with emphasis on assessment, case management, and crisis intervention.
  • Behavioral health experience.

Nice-to-haves

  • Inpatient experience.
  • Dual diagnosis experience with mental health and substance abuse.
  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients.
  • Experience with individuals involved in the justice system.
  • Knowledge of the system of care in San Diego.
  • Bilingual skills (fluency in Spanish/English).
