About the position

The Behavioral Health Consultant (BHC) provides integrated care to pediatric and adult patients as part of a Primary Care Medical Home or Specialty Care treatment team. This role focuses on brief, evidence-based assessments and interventions aimed at functional improvement, working closely with medical teams to address a variety of medical and mental health concerns. The BHC operates in a fast-paced environment, adapting to both patient-facing and physician-facing roles, and is responsible for high-volume patient visits and effective communication with healthcare providers.

Responsibilities

  • Provide expert integrated care to pediatric and adult patients as part of a treatment team.
  • Conduct brief, evidence-based assessments and interventions for functional improvement.
  • Utilize biopsychosocial models for patient assessment and identify substance abuse problems.
  • Apply first-line interventions for a broad spectrum of physical and mental health issues.
  • Educate patients and families to encourage compliance with treatment plans.
  • Discuss psychotropic medications with patients and recommend initiation to primary care physicians.
  • Utilize electronic health records for timely communication with medical providers.
  • Document patient sessions concurrently and meet billing and documentation requirements.
  • Assess and manage suicidal and homicidal risks, and concerns for abuse and neglect.
  • Provide consultation and training to primary care team members on mental health and behavioral medicine.

Requirements

  • Minimum two years of post-graduate clinical experience in an integrated care delivery system.
  • Master's degree in a mental health-related field.
  • Valid Licensed Clinical Social Worker license or equivalent within six months of hire.
  • Demonstrated ability to work in a fast-paced environment and manage multiple demands.
  • Current knowledge of cultural competency related to health and emotional well-being.

Nice-to-haves

  • Clinical experience within an integrated care delivery system.
  • Interest in health and fitness, and larger systems issues in patient care.
  • Masters or Doctoral degree with emphasis in Integrated Care.
