About the position
The Behavioral Health Consultant (BHC) provides integrated care to pediatric and adult patients as part of a Primary Care Medical Home or Specialty Care treatment team. This role focuses on brief, evidence-based assessments and interventions aimed at functional improvement, working closely with medical providers to ensure comprehensive patient care. The BHC operates in a fast-paced environment, adapting to both patient-facing and physician-facing roles, and is responsible for high-volume service delivery in a collaborative healthcare setting.
Responsibilities
- Apply knowledge of biopsychosocial models for patient assessment.
- Conduct brief interventions for a range of physical and mental health issues.
- Provide patient and family education to support treatment compliance.
- Utilize electronic health records for timely communication with medical providers.
- Assess and manage suicidal and homicidal risks, and report abuse concerns as necessary.
- Consult and train primary care team members on mental health and behavioral medicine topics.
Requirements
- Minimum two years of post-graduate clinical experience in integrated care delivery.
- Master's degree in a mental health-related field.
- Valid Licensed Clinical Social Worker license or equivalent within six months of hire.
