About the position

The Behavioral Health Consultant LSCSW at HealthCore Clinic is responsible for providing mental health and behavioral health services to individuals, families, and groups. This role emphasizes a patient-centered approach, ensuring that clients receive comprehensive care while maintaining up-to-date records. The consultant will work closely with primary care providers to manage patients with chronic emotional and health issues, offering therapy and support to promote healthy behavior changes.

Responsibilities

  • Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
  • Work with primary care and other departments to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
  • Consult with patient primary care providers to enhance understanding of the patient and assist in treatment planning.
  • Maintain a patient caseload in accordance with the needs of the Clinic.
  • Offer weekly intake appointment times as identified by the Director of Behavioral Health.
  • Use the DSM V and other assessment tools to identify and manage patients with various physical and behavioral health issues.
  • Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
  • Provide skill training through psycho-education and develop specific behavioral change plans for patients.
  • Provide individual, group, and family therapy to meet the varied needs of the patient.
  • Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
  • Provide self-management support to patients by encouraging healthy behavior changes.
  • Maintain professional and technical knowledge by attending educational workshops and participating in professional societies.
  • Provide on-call coverage in rotation with all department clinical staff as needed.
  • Prepare for meetings and court appearances and provide court testimony when necessary.
  • Complete documentation in compliance with clinic policy and governing regulations.

Requirements

  • Current and valid licensure in Social Work (LSCSW) within the state of Kansas required.
  • Master's degree in Social Work is required.
  • Practicum clinical experience required.
  • Prior experience working as a Social Worker in a clinical setting or FQHC preferred.
  • Advanced working knowledge of behavioral medicine and evidence-based treatments for mental health conditions.
  • Knowledge of special needs populations including MR/DD and SED children.
  • Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
  • Ability to communicate effectively with vulnerable populations.
  • Remain flexible to withstand unpredictable changes to workflow and schedules.
  • Ability to maintain absolute confidentiality in regards to HCC matters.

Nice-to-haves

  • Experience in community health services.
  • Familiarity with patient-centered medical home models.
