About the position

ChesPenn is a network of community health centers spanning Chester and Delaware Counties. Our mission is to provide quality, integrated, and culturally sensitive medical, dental, and behavioral healthcare to adults and children regardless of their income, insurance status, or ability to pay. We are seeking a skilled, compassionate Behavioral Health Consultant (BHC) who will work full time at our Center for Family Health at Coatesville in Coatesville, PA. The BHC serves as a member of the healthcare team assisting our Primary Care Providers in managing the overall health of their patients through a HYBRID MODEL that consists of in-office visits (3 days) and virtual visits (2 days). The goals of our BHC staff are to assist in improving recognition, treatment, and management of mild psychosocial/behavioral problems and medical conditions in the patient population.

Responsibilities

  • Make targeted assessments and evaluations, including diagnostic impressions and functional status, focused on the presenting problem
  • Provide timely and succinct feedback to Providers regarding consultation findings and recommendations
  • Enter concise documentation in the patient's electronic medical record
  • Triage and refer patients to specialty mental health care when appropriate
  • Formulate behavioral health interventions appropriate to the primary care setting, and assisting with implementation of patient treatment plans
  • Provide brief follow-up, including relapse-prevention education
  • Develop, teach, and/or provide oversight for classes that promote education and skill-building to enhance psychological and physical health
  • Provide on-going consultation services for a subset of patients who require on-going monitoring and follow-up
  • Share knowledge with other team members and patients, both formally (in-services, consult responses) and informally (hallway conversations)
  • Work as a primary care team member to develop specific clinical pathways or best practice programs for targeted patient groups (e.g., patients with chronic health conditions)

Requirements

  • Licensed Clinical Social Worker, Behavioral Health Nurse Practitioner, Licensed Clinical Psychologist, Licensed Professional Therapist, or Licensed Marriage & Family Therapist
  • Minimum 2 years' experience
  • Licensed to work in Pennsylvania

  • Ability to work harmoniously as part of a healthcare team
  • Effective and efficient computer skills including spreadsheet, word, and EHR
  • Positive, enthusiastic and tolerant demeanor while directly working with patients and healthcare professionals
  • Strong influencing skills and perseverance
  • Self-motivated and able to work independently
  • Ability to define problems, collect data and analyze results
  • Possession of a valid driver's license and a licensed automobile preferred
  • Ability to work effectively as a team player.
