About The Position

The Integrated Behavioral Health Consultant (BHC) plays a crucial role within the Primary Care team at Waterfall Community Health Center (WCHC), providing trauma-informed and evidence-based care under the supervision of the Mental Health Program Manager. This position is designed to address the social, behavioral, and emotional factors that impact patients' physical health through short-term counseling and consultation. The Integrated BHC is responsible for delivering a wide range of comprehensive behavioral services, which include conducting screenings, providing psychoeducation, making brief assessments, discussing interventions, and offering referrals. The consultant will also contribute to care coordination for individuals, families, and groups across various age groups. In this role, the Integrated BHC is expected to adhere to WCHC's mission and core values, which emphasize Integrity, Respect, Accountability, Compassion, Partnership, and Collaborative Communication. The consultant will provide trauma-informed counseling services within a Primary Care setting, which involves performing brief assessments and interventions, facilitating both individual and group counseling, and coordinating with community partners. Accurate and timely documentation of all patient interactions is essential, in compliance with clinic policies and relevant guidelines. The Integrated BHC will work independently while effectively communicating with patients, coworkers, and supervisors. Building positive rapport with a diverse patient population, including those with developmental conditions, psychiatric diagnoses, and substance use issues, is a key aspect of this role. The consultant will develop and implement treatment strategies with measurable objectives and collaborate with Psychiatric and Primary Care Providers to address mental health and psychosocial issues. Participation in team meetings and daily huddles is required to review the care of complex patients, particularly those whose behavioral health needs contribute to long-term health conditions. The BHC will also manage patient communications through various channels and adapt communication strategies to de-escalate high-stress situations, ensuring a safe and confidential environment in accordance with HIPAA standards. Additionally, the consultant will support program goals aimed at reducing cultural and socio-economic barriers for marginalized and diverse community populations, while also pursuing professional development through supervision and training.
