About the position

The Behavioral Health Consultant at Adelante Healthcare is responsible for managing the psychosocial aspects of chronic and acute diseases, applying behavioral principles to address health risks and lifestyle issues. The role emphasizes prevention, self-help approaches, and collaboration with patients to foster personal responsibility for health. The consultant provides consultation and co-management for mental disorders and psychosocial issues within a multidisciplinary primary care model.

Responsibilities

  • Provide behavioral health services to children and families as part of the continuum of care
  • Provide quality clinical consultation to providers and staff regarding mental and behavioral health issues
  • Conduct assessments and diagnostics for patients referred by medical staff
  • Deliver brief, evidence-based, solution-focused behavioral interventions
  • Cultivate education and self-management skills for targeted populations
  • Promote knowledge of patient behavioral health issues among practitioners
  • Facilitate collaboration between care provided and community mental health systems
  • Provide on-site availability for crisis triage and intervention
  • Maintain an active presence in case staffing and clinical huddles
  • Document clinical behavioral health interventions and track patient referrals
  • Attend monthly department staffing and weekly supervision meetings
  • Promote positive patient relations and comply with organizational policies
  • Maintain confidentiality of patient and employee information
  • Participate in interdisciplinary communication and collaboration

Requirements

  • Master's degree in the Behavioral Health field (Psychology, Social Work)
  • Two years of directly related experience
  • Computer literacy
  • Certification or certification eligibility as an LCSW, Ed.D, Psy.D., LPC, or Ph.D.
  • Excellent knowledge of behavioral medicine and evidence-based treatments
  • Ability to work through brief patient contacts and make quick clinical assessments
  • Comfortable with the pace of primary care and working in an interdisciplinary team
  • Good knowledge of psycho-pharmacology
  • Ability to design and implement clinical pathways for chronic conditions
  • Certification in CPR and AED
  • Level One Fingerprint clearance card issued by Arizona Department of Public Safety

Nice-to-haves

  • Bilingual (English/Spanish) preferred
  • Additional experience in behavioral health
