About the position
The Behavioral Health Consultant will advocate for and assist patients in accessing essential resources to address their medical and behavioral health concerns. This role involves providing personalized recommendations and implementing behavioral interventions to support patients' wellness journeys. The consultant will work closely with the Integrated Medical Team to ensure a comprehensive approach to healthcare, contributing to the mission of delivering holistic care to the community.
Responsibilities
- Advocate for assigned patients to access vital resources for medical and behavioral concerns.
- Offer personalized recommendations to support patients' wellness.
- Implement behavioral interventions to assist patients in their recovery journey.
- Collaborate with the Integrated Medical Team to provide comprehensive healthcare solutions.
Requirements
- Master's degree in Social Work or related field.
- Valid Licensed Clinical Social Worker (LCSW) license or equivalent (LMHC, LMFT, LCAC).
- 1-2 years of post-degree clinical experience.
- Basic computer skills including Microsoft Office and EHR systems.
Nice-to-haves
- Ph.D. and APA certification (not required but preferred).
- Additional primary-care experience (not required but preferred).
