About the position

As a Behavioral Health Provider (BHP) at Valley-Wide Health Systems, you will play a vital role in supporting the mental and emotional well-being of patients, including children, adolescents, and adults, through integrated, team-based care. This position embodies the mission of our FQHC, delivering biopsychosocial care to improve the health and well-being of individuals and families in our community.

Responsibilities

  • Participates in activities/workflows to identify 'at risk' patients that consider individual patient needs in order to prevent worsening of their condition.
  • Evaluate crisis and apply appropriate interventions.
  • Provide diagnostic mental health assessments and screenings utilizing standardized instruments.
  • Work with providers, patients, and their families to formulate biopsychosocial treatment plans.
  • Provide evidence-based interventions, and/or psychotherapy, for clients who need Behavioral/Mental Health services.
  • Initiates follow-up to monitor client progress and communicates with treatment team.
  • Accurate and timely documentation in patients' chart, utilizing approved EHR workflow.
  • Consultation with treatment team to enhance understanding of the patient and assist in the implementation and monitoring of treatment plans.
  • Assist the treatment team during integrated visits.
  • Develops, as appropriate, relapse prevention plans and helps patients maintain stable functioning.
  • Collaborate with internal and external partners in support of the individual/family's growth and development.
  • Connect patients to Enabling Services who assist with Social Determinants of Health.

Requirements

  • Actively pursuing a Master's or Doctoral degree in psychology, counseling, or clinical social work with a minimum of 1 year of schooling completed.
  • Candidates may hold a current license with the Colorado State Board as a Licensed Clinical Psychologist (Ph.D., PsyD), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).
  • Must be able to successfully complete drug testing and background screening.

Nice-to-haves

  • Bilingual skills (English/Spanish) desirable but not required.
