About the position

This individual will work in collaboration with primary care physicians to provide integrated medical/behavioral health services to patients. Interventions will be brief, understandable, effective, and consistent with a population-based model of care. This position will fill in at our Bryan, Defiance, Toledo, and Tiffin Health Centers.

Responsibilities

  • Provides quality clinical consultation addressing patient psycho-social concerns in collaboration with the integrated primary care team.
  • Provides brief, health and behavior focused assessments and interventions with active development of behavioral change planning.
  • Assesses patient's mental health related conditions utilizing DSM V, SBIRT and other relevant screening and diagnostic tools.
  • Cultivates education and self-management skills to targeted populations with comorbid medical and mental health conditions.
  • Utilizes a variety of intervention approaches including Motivational Interviewing, Solution focused therapy, Cognitive Behavioral Therapy, and others.
  • Provides education to the primary care team regarding integrated behavioral health assessment and intervention.
  • Establishes and builds collaborative relationships with medical providers.
  • Promotes smooth interface between medicine and behavioral health care throughout the organization.
  • Facilitates referrals to specialized mental health providers and psychiatric services as needed.
  • Provides patient visits via Telemedicine/Telehealth.
  • Maintains accurate electronic patient records while complying with HIPAA and Confidentiality policies.
  • Develops treatment plans appropriate for primary care behavioral health patients.
  • Attends applicable provider meetings and participates in ongoing training activities.
  • Collaborates and co-manages the patient's behavioral health care with consistent feedback to collaborating providers.
  • Documents patient assessment/intervention data in health records.
  • Provides patient education about health maintenance and management of chronic disease processes.
  • Establishes an interpersonal relationship with individuals or families to enhance their capacity for self-care.
  • Participates as a member of a multidisciplinary team to facilitate teamwork.
  • Demonstrates clear and effective communication skills.
  • Maintains professional competence through continuous learning.
  • Complies with personnel, behavioral health, medical and clerical policies and procedures.
  • Adheres to standards and policies of Organizational Privacy/Security and other regulations.
  • Performs other duties as assigned.

Requirements

  • Master's Degree in a Behavioral Health field (Psychology, Counseling, or Social Work).
  • Licensed in the State of Ohio.
  • Successful completion of in-house training.
  • Two years of directly related experience.
  • Computer literacy and experience.
  • Ability to work as a team member.
  • Ability to work with a culturally diverse group of people.
  • Willingness to work flexible hours.
