About the position

The purpose of this job is to improve the overall health of the communities served by providing social or behavioral health services to patients and families. The role involves performing various duties including individual and group counseling, collecting psycho-social data, conducting mental health evaluations, developing care plans, linking clients with community resources, and participating in community outreach activities. Additionally, staff with chemical dependency expertise will perform assessments and provide counseling focused on substance use treatment. The position may also involve supervising volunteers and students, facilitating peer reviews, and supporting the CHAS Health Mission and Core Values.

Responsibilities

  • Provides individual and group counseling services within individual scope of practice.
  • Available to primary care providers for behavioral health staffing and consultation.
  • Collects psycho-social data about individuals and provides gathered information to providers.
  • Performs mental health evaluations utilizing approved evaluation tools.
  • Develops detailed care plans for each client and documents as part of the electronic health record.
  • Links clients with various community resources.
  • Provides in-service training for CHAS staff as requested.
  • Participates in community outreach activities as directed.
  • Responsible for performing chemical dependency assessments on clients.
  • Conducts individual and group counseling and education focusing on alcohol and drug treatment.
  • Supervises and coordinates volunteers and behavioral health/social work students as required.
  • Facilitates peer review and chart audit process as assigned.
  • Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Requirements

  • Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Marriage and Family Therapist (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor required.
  • Two years of clinical practice with underserved populations desired.
  • Knowledge of available community linkage resources.
  • Excellent oral and written communications skills required.
  • Computer skills required.
  • Work requires consistent exercise of independent judgment and discretion.
  • BLS (CPR/AED) required.
  • Valid drivers' license and insurance required.
