About the position

The Behavioral Health Provider at Summit Medical Consultants is responsible for delivering compassionate and coordinated mental health care to participants in an integrated care setting. This role involves conducting mental health assessments, providing therapeutic interventions, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. The position requires adaptability, strong interpersonal skills, and a commitment to professional development.

Responsibilities

  • Completes mental health assessments and documents in the EMR, including diagnosis, recommendations, and team-based care planning.
  • Provides solution-focused interventions to participants and/or caregivers, including monitoring and crisis intervention.
  • Consults with team members regarding clinical findings and participates in case conferencing.
  • Collaborates with interdisciplinary teams and external providers to support participants at risk for hospitalization.
  • Monitors responses to psychotropic medications and coordinates with psychiatric consultants for continuity of care.
  • Educates the interdisciplinary team on managing behavioral complexities and placements.
  • Liaisons with external psychiatric providers for appropriate referrals and continuity of care.
  • Participates in interdisciplinary team meetings, conferences, committees, and events as requested.

Requirements

  • Master's degree in behavioral health, social work, or psychology.
  • 1+ year working in an integrated care setting.
  • Licensed Mental Health Professional (LCSW, PsyD, PhD, LMFT, LPC).
  • Current CPR certification or ability to obtain within the first 30 days of employment.

Nice-to-haves

  • Doctoral degree in clinical psychology.
  • Experience in integrated primary care settings.
  • Knowledge of geropsychiatric populations.
  • Training or experience with group and family interventions.
  • Experience with conflict management, mediation, de-escalation, and crisis mitigation.
  • Knowledge of chronic disease management and its psycho-social implications.
  • Exceptional organizational skills.
  • Valid driver's license with a clean driving record.
