About the position

The Behavioral Health Provider role at The Healthcare Connection is a unique opportunity to work in a school-based health center, focusing on providing quality care and access to resources for patients. Responsibilities

  • Utilize a brief consultation model to communicate with primary care providers.
  • Support patients and families in self-management and behavior change through a team-based care approach.
  • Provide consultation and training to primary care team members on mental health and behavioral medicine.
  • Assist in the implementation and monitoring of biopsychosocial treatment plans.
  • Manage psychosocial aspects of chronic and acute diseases in collaboration with primary care providers.
  • Develop clinical pathways or best practice programs for targeted patient groups.
  • Identify and facilitate referrals for further assessment and services as needed.
  • Provide behavioral health services and substance abuse screening to various age groups.
  • Conduct comprehensive bio-psychosocial assessments for mental health and substance abuse issues.
  • Document all patient interactions using electronic medical records and tracking tools.
  • Be available for crisis intervention and psychiatric emergencies during working hours.

Requirements

  • Master's degree in clinical social work, counseling, or psychology.
  • Licensed in the State of Ohio (supervisory hours can be provided).
  • Three or more years of related experience in population-based healthcare principles.
  • Excellent knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Experience working with ethnically and culturally diverse families.
  • Bilingual in Spanish preferred.
  • Experience working with pediatric and adult patients.

Nice-to-haves

  • Experience in school-based health settings.
  • Familiarity with electronic medical records systems.
