Responsibilities
- Utilize a brief consultation model to communicate with primary care providers.
- Support patients and families in self-management and behavior change through a team-based care approach.
- Provide consultation and training to primary care team members on mental health and behavioral medicine.
- Assist in the implementation and monitoring of biopsychosocial treatment plans.
- Manage psychosocial aspects of chronic and acute diseases in collaboration with primary care providers.
- Develop clinical pathways or best practice programs for targeted patient groups.
- Identify and facilitate referrals for further assessment and services as needed.
- Provide behavioral health services and substance abuse screening to various age groups.
- Conduct comprehensive bio-psychosocial assessments for mental health and substance abuse issues.
- Document all patient interactions using electronic medical records and tracking tools.
- Be available for crisis intervention and psychiatric emergencies during working hours.
Requirements
- Master's degree in clinical social work, counseling, or psychology.
- Licensed in the State of Ohio (supervisory hours can be provided).
- Three or more years of related experience in population-based healthcare principles.
- Excellent knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Experience working with ethnically and culturally diverse families.
- Bilingual in Spanish preferred.
- Experience working with pediatric and adult patients.
