About the position

The Bilingual C-Kin Kinship Navigator at Family & Children's Agency Inc. is a full-time role focused on supporting kinship caregivers in raising resilient children. This position involves conducting assessments, creating care plans, and connecting families with community resources. The C-KIN program aims to provide equitable support and build self-reliance among kinship families through collaboration and advocacy.

Responsibilities

• Conduct assessments and create care plans for families.
• Partner with kin families to identify needs and set goals.
• Respond to family referrals and ensure timely follow-up.
• Assist families in accessing available resources and services.
• Maintain contact with families post-discharge to ensure stability.
• Document case details accurately using the Electronic Health Record system.
• Collaborate with other experts to provide holistic support.

Requirements

• Bachelor's degree in a related field or equivalent experience.
• Minimum of 2 years of lived kinship caregiving experience.
• Prior experience in social services or a related field.
• Bilingual in English and Spanish is a plus.
• Valid driver's license and access to a personal vehicle.
