About the position

The Bilingual Case Manager at Exceptional Children's Foundation is responsible for providing case management services to adults with varying degrees of learning, behavioral, emotional, and developmental disabilities. This role involves maintaining case records, advocating for consumers, and coordinating services to promote independence and well-being. The position requires strong communication skills and the ability to work collaboratively with consumers, families, and service providers.

Responsibilities

  • Develop, prepare, and maintain complete case records and documentation for all referrals and consumers according to standards.
  • Consult with program staff to create individual service plans for consumers.
  • Establish and monitor consumer goals and objectives on a regular basis.
  • Participate in the intake and assessment process for new consumers.
  • Assist in developing and implementing behavior management programs and provide crisis intervention as needed.
  • Monitor consumer attendance and progress, collaborating with staff on evaluations.
  • Conduct annual meetings with consumers, families, and regional center representatives.
  • Provide in-service training to other staff and consumers regarding safety and self-advocacy.
  • Keep abreast of community resources and advocate for consumers' needs.

Requirements

  • Bachelor's degree in a Human Service Field.
  • Two years of experience working with individuals with special needs or one year as a Case Manager in a social service environment.
  • Excellent verbal and written communication skills.
  • Computer literate, particularly in MS Word and Excel.
  • Detail-oriented with a high degree of accuracy.
  • Valid California driver's license and reliable transportation.

Nice-to-haves

  • Bilingual in Spanish preferred.
  • Experience in crisis intervention and behavior management.
