About the position
The Bilingual Case Manager at Care Plus NJ, Inc. is responsible for providing treatment and support to individuals experiencing homelessness. This role involves outreach to unserved community members, including those with serious mental illness and co-occurring disorders, and working in various community locations to ensure they receive the necessary care and resources. The case manager acts as a first responder to complex crises, develops care plans, and links clients to essential community resources.
Responsibilities
- Act as a 'first responder' to complex crises involving psychological and environmental components.
- Work holistically to ensure care and discharge plans meet the physical, social, and emotional needs of clients.
- Assist individuals with housing assistance.
- Complete comprehensive assessments of clients' needs and identify resources.
- Document all aspects of services provided and create care plans with clients.
- Link clients to community resources as needed.
Requirements
- Bachelor's degree in health, psychology, counseling, social work, education, or other behavioral health profession.
- 1-2 years of experience working with the homeless population or high-need individuals with serious mental illness, co-occurring disorders, or substance use disorders.
- Knowledge, skill, and experience necessary to competently perform case management activities.
- Valid driver's license and insurable by the agency's automobile insurance.
