About the position

The Bilingual Community Health Worker (CHW) at Oak Orchard Health plays a crucial role in improving health access and conditions within the community. This position serves as a liaison between patients and healthcare providers, advocating for individuals with unmet needs and assisting them in navigating the healthcare system. The CHW is responsible for providing culturally and linguistically responsive healthcare, ensuring that patients are actively engaged in their health management.

Responsibilities

  • Maintain strict confidentiality while sharing information with appropriate staff, adhering to HIPAA guidelines.
  • Provide interpretation for Spanish-speaking patients during interactions with staff and appointments.
  • Utilize roles such as medical interpreter, cultural broker, and advocate for patients.
  • Conduct needs assessments through informal conversations or standardized screening tools.
  • Identify barriers patients may face, such as low literacy, and work to reduce these barriers.
  • Link patients to local community resources for financial hardship and basic needs.
  • Assist patients in accessing health-related services and making appointments.
  • Support Case Managers in following up on caseloads and communicating with patients.
  • Register new patients and schedule appointments as needed.
  • Assist patients with prescription renewals and paperwork for health services programs.
  • Provide transportation for patients to appointments and community resources.
  • Work with community agencies to promote health awareness and disease prevention programs.
  • Develop relationships to reduce barriers for at-risk patients.
  • Provide educational materials in appropriate languages and literacy levels.
  • Travel to area worker camps to promote the center and recruit patients.

Requirements

  • Bilingual in Spanish and English preferred.
  • Excellent customer service skills.
  • Strong teamwork skills.
  • Digital literacy, including proficiency in Microsoft programs and internet browsers.
  • Ability to build rapport with the agricultural worker community.
  • Flexibility in scheduled hours and work sites.

Nice-to-haves

  • Experience in human or health services fields.
  • Education or certification in human or health services preferred.
  • Current New York State driver's license.
