About the position

The Bilingual Community Services Case Manager at Catholic Charities of the Diocese of Fort Wayne-South Bend is responsible for providing essential support and case management services to individuals and families in need. This role focuses on assisting clients with pregnancy support, rent and utility assistance, human trafficking victim support, and homeless services. The case manager will work to connect clients with resources that promote self-sufficiency and improve their overall situations.

Responsibilities

  • Provide case management services and related support to adults and children in various programs.
  • Communicate effectively with clients from diverse backgrounds.
  • Conduct assessments, case planning, advocacy, referral, crisis management, and skills development.
  • Assist pregnant and parenting teens in overcoming barriers to education.
  • Provide direct services for emergency financial or material assistance.
  • Conduct skills building or training sessions for clients.
  • Manage case services for residents transitioning out of the Center for the Homeless.
  • Coordinate activities for clients impacted by disasters.
  • Develop and maintain relationships with community service providers.
  • Promote and represent programs in the community.
  • Attend community service trainings and meetings.
  • Ensure accurate case documentation and records.
  • Assist in data collection and preparation of grants and program reports.

Requirements

  • Bachelor's degree in social service or related field from an accredited college/university.
  • Minimum of one year work experience in a social service setting.
  • Bilingual in Spanish and English required.
  • Working knowledge of community service and public program providers.
  • Knowledge of CORE 40/HSE requirements and FAFSA application process.
  • Knowledge of employment skills development and financial literacy.
  • Ability to work independently with limited supervision.
  • Excellent written and oral communication skills.
  • Valid driver's license with a clean driving record.
  • Computer literacy in Microsoft Office 365.

Nice-to-haves

  • Experience in crisis management.
  • Familiarity with budgeting and financial literacy programs.
