About the position

The Home Care Coordinator at AlfaBEST Home Care is a vital role responsible for coordinating and maintaining care for clients. This position requires a bilingual individual fluent in both English and Spanish, as effective communication with clients and their families is essential. The coordinator will build and maintain relationships with referral sources, ensuring that clients receive the best possible care tailored to their needs. The role involves understanding and adhering to established policies and procedures, as well as comprehending all programs and services offered by the agency. The coordinator will also be responsible for resolving client complaints and communicating any changes in client conditions to program case managers. In addition to client interaction, the Home Care Coordinator will assist in scheduling requests for caregivers and clients, perform home visits as needed, and participate in local, state, and national health committees relevant to home care. The position requires maintaining accurate client and staff databases, tracking progress and results of completed work, and performing general office duties such as word processing, filing, and providing reception services. The coordinator will also be involved in promoting positive community relations through participation in community affairs and committees. This role is crucial in ensuring that clients receive the necessary services as outlined in their care plans and agreements. The Home Care Coordinator must be a self-starter, motivated, and able to handle the demands of the job with maturity and professionalism. The position offers a starting salary between $45,000 and $55,000 annually, with additional bonuses for on-call duties.

Responsibilities

  • Understand and adhere to established AlfaBEST Home Care policies and procedures.
  • Create and maintain solid client relationships by handling questions and concerns with speed and professionalism.
  • Discern client services required as outlined in agreements, urgent requests, and care plans.
  • Comprehend all programs and services offered by the agency.
  • Understand and comprehend all State home care regulations with guidance from the Administrator.
  • Resolve client complaints effectively.
  • Communicate all client needs and changes of condition to program case managers.
  • Assist in daily Caregiver and Client scheduling requests.
  • Perform home visits as needed.
  • Participate in local, state, and national health committees relevant to home care.
  • Promote positive community relations through involvement in community affairs, boards, and committees.
  • Enter client information into the database accurately.
  • Maintain staff and client database including EMR/EHR, dates and reminders of TB, COVID-19, work documentation, Performance Evaluations, Continuing Education, client admission, and hospital admission/discharge.
  • Maintain staff and client statistics and reports.
  • Track progress and results of completed work.
  • Perform general office duties including word processing, photocopying, filing, shredding, sorting/distributing mail, and providing reception and telephone services.
  • Cover priority clients as needed.
  • Participate in on-call rotation and handle ad hoc projects as needed.

Requirements

  • Bilingual in Spanish and English.
  • High School Diploma or GED.
  • Prior experience in Home Care/Home Health in an office setting is required.
  • Proficient in computer skills including MS Office, MS Excel, and scheduling programs.
  • Basic knowledge of medical terminology.
  • Excellent interpersonal, organizational, and communication skills.
  • Ability to carry out directions and read/write with minimal supervision.
  • Maturity and ability to deal effectively with job demands.
  • Self-starter, motivated, and energetic personality.
  • Ability to troubleshoot, prioritize, and present a polished, positive image to co-workers, care providers, clients, and families.
