About the position

The Care Management Trainer and Auditor is responsible for providing training and auditing for the Care Management team, ensuring compliance with departmental goals and quality standards. This role involves overseeing complex cases, maintaining documentation, and collaborating with management to improve workflows and training materials. The position requires a strong background in healthcare and care management, along with the ability to effectively train and audit staff.

Responsibilities

  • Provides training and auditing for Care Management team.
  • Completes team and individual training for new and existing Care Management department employees.
  • Completes ongoing audits for the department as per current initiatives.
  • Participates in quality and compliance projects/initiatives in support of departmental goals.
  • Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed.
  • Provides critical oversight for complex cases.
  • Back-up to the Directors of Care Management and Care Managers as needed.
  • Collaborates with Directors of Care Management to review, update and improve workflows and materials to maximize opportunities for quality and compliant Care Management.
  • Assists Directors of Care Management with Care Management related tasks/initiatives.
  • Performs other duties as assigned.

Requirements

  • BSN, RN, MSW or related degree
  • 5 years satisfactory related experience; experience in Health Care, desired.
  • Proficient in the use of office equipment and the computer; EHR, Microsoft Word and Excel.
  • Self-directing, good organizational and interpersonal skills.
  • Ability to effectively train, oversee, and audit employees in a professional manner.

Nice-to-haves

  • Experience in Care Management: 2 years (Preferred)
  • Experience with Microsoft Excel: 1 year (Preferred)
  • Ability to commute to Brooklyn, NY 11218 (Preferred)
  • Ability to relocate to Brooklyn, NY 11218 before starting work (Required)
