About the position

The Bilingual RN Healthcare Manager at WellMed is responsible for supporting patients with high-risk health conditions in navigating the healthcare system. This role emphasizes patient empowerment through education, resource provision, and advocacy, ensuring a high quality of life for patients and their families. The Healthcare Manager collaborates with multidisciplinary care teams and acts as a resource for clinic staff, performing nursing duties within the scope of an RN license in a self-directed environment.

Responsibilities

  • Identify patients at high risk in collaboration with providers and clinic staff.
  • Support longitudinal care of patients with chronic conditions through assessments and medication reconciliation.
  • Conduct Motivational Interviewing and Self-Management Goal setting.
  • Provide patient education and create referrals to appropriate agencies and resources.
  • Assist in the transition of patients from inpatient to outpatient settings by assessing transitional needs and establishing contingency plans.
  • Perform accurate and timely documentation in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare accurate and timely reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.

Requirements

  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Proven knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills to work efficiently with electronic medical records.
