About the position

The Blended Case Manager at RHA Health Services plays a crucial role in supporting individuals with intellectual and developmental disabilities, as well as those facing mental health and substance use challenges. This position involves building a strong network of community resources and services to assist clients in accessing essential living needs and skills. The Blended Case Manager advocates for clients, helps navigate complex systems, and provides crisis intervention, ensuring that clients receive the necessary support 24/7.

Responsibilities

• Advocate for clients and assist in negotiating complex systems such as special education and social services.
• Help clients maintain and access basic living needs such as employment, housing, food, medical care, and recreation.
• Provide crisis intervention and on-call case management support 24/7.
• Develop service plans, treatment plans, and individual crisis plans based on clients' strengths and needs.
• Prepare and complete all necessary documentation, including assessments, progress notes, and incident reports within required timeframes.
• Assist clients in gaining funding authorization for needed services and entitlements.
• Collaborate with county Office of Mental Health/Intellectual Disability personnel and other service providers to assess consumer needs and provide appropriate services.

Requirements

• Bachelor's degree in a human services related field (sociology, social work, psychology, etc.) or an Associate's Degree with relevant college credits and two years of experience in the mental health/human service field.
• Valid driver's license.
• Excellent verbal and written communication skills.
• Ability to access community resources and assist consumers in gaining access to services.
• Problem-solving skills to assist consumers with resolution of issues.
• Interpersonal skills to develop and enhance consumer networks.
