About the position
The Blended Case Manager role at Elwyn involves providing outreach and support to individuals with serious and persistent mental illness in their home settings. The primary focus is to help clients access, coordinate, and monitor necessary services while advocating for their needs. This position aims to maintain individuals in the least restrictive environment, promoting independence and mental health stability through a person-centered approach.
Responsibilities
- Preparing and maintaining timely, accurate, and complete records
- Participating as a team member to improve the overall effectiveness of the work group and achieve group goals
- Ensuring interventions and supports are delivered consistently, flexibly, and responsively to changing client needs
- Involving clients in identifying and coordinating treatment issues
- Empowering clients through therapeutic rapport, advocacy, and skill building
- Exhibiting proficiency with benefit and entitlement options to maximize treatment planning opportunities
- Coordinating emergency psychiatric services in a timely manner, including documenting interventions and responses from clients and supporting agencies.
Requirements
- Bachelor's Degree preferred in Human Services field (social work, psychology, child development, special education, rehabilitative counseling, or closely related field)
- A commitment to community-based mental health services
- Experience with case management strongly encouraged but not required
- Computer literacy required
- Knowledge of non-violent crisis intervention principles and practices
- Knowledge of the cognitive and behavioral aspects of mental illness
- Valid driver's license and record in good standing.
