About the position
The CAPE Clinical Social Worker role is a full-time position focused on providing high-quality, coordinated care for children requiring long-term ventilator assistance and their families. The position involves conducting psychosocial assessments, managing cases, and collaborating with a multidisciplinary team to ensure comprehensive support for patients and their families.
Responsibilities
- Conducting psychosocial assessments and providing consultation and support to families.
- Managing cases and coordinating complex resources with community, governmental, school, healthcare, and other agencies.
- Collaborating with a multidisciplinary team across the continuum of care.
Requirements
- Masters in Social Work and state of MA LICSW.
- Several years of social work experience in a medical environment with chronic illness highly preferred.
- Ability to thrive in a fast-paced, dynamic environment, along with flexibility and excellent communication skills.
Nice-to-haves
- Bilingual in English and Spanish is preferred.
