About the position

The Brigaide Mobile Outreach Case Manager at Metropolitan Ministries is responsible for building relationships with individuals experiencing homelessness and providing advocacy to connect them with essential services and housing resources. This role involves direct outreach, case management, and collaboration with community partners to facilitate access to various support services, ultimately aiming to reduce homelessness in the Tampa Bay area.

Responsibilities

• Build relationships with individuals experiencing homelessness through outreach efforts.
• Provide advocacy and assist clients in connecting with housing services and resources.
• Identify unsheltered homeless individuals through referrals and direct outreach activities.
• Maintain an ongoing caseload of clients, ensuring documentation is uploaded into the Homeless Management Information System (HMIS).
• Participate in Coordinated Entry meetings to address barriers in the process.
• Collaborate with other service providers to facilitate access to basic needs and housing assistance.
• Conduct assessments and maintain client records, including daily activity logs and reports.
• Encourage client participation in activities that promote self-sufficiency.
• Respond to incoming calls and provide telephonic services as needed.
• Drive a van/shuttle for outreach activities, ensuring safety and maintenance of the vehicle.

Requirements

• Bachelor's degree in social services or a related field.
• Minimum of two years of case management or social services experience.
• Thorough knowledge of community resources and data organization.
• Effective communication skills and ability to work with diverse populations.
• Strong leadership skills and ability to maintain professional boundaries.
