• Utilize technology for case management using Wellsky/Axxess.
• Open cases using Oasis software after training.
• Collaborate with healthcare teams to ensure quality patient care.
• Document patient progress and care plans accurately.

Requirements

• Registered Nurse (RN) license required.
• Home care experience within the last 10 years.
• Tech-savvy with experience using tablets for documentation.
• Ability to learn and adapt to new software and systems.

Nice-to-haves

• Bilingual skills are a plus.
• Previous experience in a Medicare agency.
