About the position

The Investigator / Case Worker position with Liberty Healthcare and the Pennsylvania Department of Human Services focuses on investigating reports of abuse, neglect, exploitation, or abandonment of adults with disabilities. The role involves conducting investigations, documenting findings, and managing active cases, all while ensuring the safety and well-being of vulnerable individuals in Butler County and surrounding areas.

Responsibilities

  • Conduct investigations in residences throughout Butler County and surrounding areas.
  • Determine if reports of abuse, neglect, or exploitation are substantiated.
  • Document findings and develop plans to mitigate risks.
  • Manage active cases and provide ongoing support to clients.
  • Travel to various locations as required for investigations.

Requirements

  • Bachelor's degree preferred, or relevant experience in behavioral health, mental health, or intellectual and developmental disabilities services.
  • Experience in field-based healthcare or human services roles.
  • Background in case management or protective services.
  • Knowledge of Social Assistance Management Systems (SAMS) and Home and Community Service Information Systems (HCSIS) is helpful.
  • Spanish speaking skills are a plus.

Nice-to-haves

  • Experience in Child, Family and Youth Services case management.
  • Law enforcement experience.
  • Service Coordinator or Program Specialist experience.
