About the position

The CAP Case Manager/Social Worker position at RHA Health Services is a part-time role focused on providing essential case management services to medically fragile children at risk of institutionalization. The role involves coordinating care with registered nurses and ensuring the health, safety, and well-being of clients in their primary residences. The position requires a compassionate individual who can effectively communicate and collaborate with clients, families, and various community agencies to develop and implement individualized care plans.

Responsibilities

  • Establish eligibility for services and obtain client histories.
  • Perform initial assessments and develop a plan to provide basic services.
  • Explain available services and programs to clients.
  • Refer clients to appropriate programs/agencies for additional needed services.
  • Evaluate and document client programs and progress.
  • Conduct interviews and work closely with clients, families, and other stakeholders to gather information for social histories.
  • Provide basic case management services as part of a Social Worker/RN interdisciplinary team.

Requirements

  • Graduation from an accredited School of Nursing.
  • Ability to direct and carry out prescribed medical treatments and therapeutic programs.
  • Thorough knowledge of professional nursing theory and practice.
  • RN certification with one year of nursing experience preferred.
  • Valid NC driver's license and a clean driving record.
  • Personal auto liability insurance if transporting clients.
